USMLE Timeline Quick Summary
You can take Step 1 and Step 2 CK in any order, while still a medical student or after graduation. Step 3 comes only after you pass both, hold your MD (or equivalent) degree and, as an IMG, have ECFMG Certification. To enter the residency Match, you must meet all ECFMG exam requirements, including OET Medicine, by the Match rank order list deadline.
When should I start the USMLE?
There is no single official age or year for the United States Medical Licensing Examination (USMLE). The official rules only fix the order of Step 3 and the limits on attempts. Within those rules, the best timeline is the one that lets you sit each Step well prepared and finish ECFMG Certification before the residency cycle you are aiming for. Most Pakistani students begin once their basic science years are complete and the concepts are still fresh.

Matching your USMLE timeline to the residency Match
Your exams need to fit the residency application cycle. The table shows the official dates for the current cycle, for residencies starting in 2027. Later cycles follow a similar pattern, so use it to plan backwards.
Because programs start reviewing applications in September, many IMGs aim to have their Step 2 CK result before then, so it appears on their application.
A sample USMLE plan
- Check that your medical school is listed in the World Directory of Medical Schools with an ECFMG Sponsor Note.
- Prepare for Step 1 once your basic science subjects are complete, and book it through the FSMB when you are consistently ready.
- Take Step 2 CK during or after your clinical years. You may take it before Step 1 if that suits you better.
- Take OET Medicine and apply for an ECFMG Pathway in time for your chosen Match cycle.
- Apply through ERAS in September and register for the NRMP Match.
- Take Step 3 in the US once you hold your ECFMG Certificate, usually before or during your first year of residency.

> Can I take USMLE Step 1 while still in medical school?
Yes. Students officially enrolled in a medical school listed in the World Directory as meeting ECFMG eligibility requirements can take Step 1 and Step 2 CK, as can graduates.
> Do I have to take Step 1 before Step 2 CK?
No. Step 1 and Step 2 CK can be taken in any sequence. Only Step 3 has to come after passing both.
> When can an IMG take USMLE Step 3?
After passing Step 1 and Step 2 CK, obtaining an MD or equivalent degree, and receiving ECFMG Certification. Step 3 is offered only in the United States and its territories.
> How soon can I retake a USMLE Step?
You can take the same Step no more than three times in 12 months. A fourth attempt must be at least 12 months after your first and six months after your most recent attempt.
> By when must I pass the USMLE to enter the Match?
You must meet all exam requirements for ECFMG Certification, including OET Medicine, by the NRMP rank order list certification deadline. For the 2027 Match, that deadline is March 3, 2027.
