USMLE exam for medical students and graduates
The United States Medical Licensing Examination (USMLE) is the three-step exam that US state medical boards use to license doctors, and the first requirement for international medical graduates (IMGs) applying to US residency programs. It tests not only what the right answer is, but why — most questions are clinical vignettes that ask you to apply basic and clinical science to real patient scenarios.
INTERFACE has been preparing students for USMLE Step 1 and Step 2 CK for more than a decade, with candidates from the Middle East, Pakistan, India, Bangladesh, Sri Lanka, the UK, Europe, and the USA. Our USMLE preparation course is taught as live online classes, running 50 to 100 hours over 5 to 10 weeks depending on your pace and schedule. Refresher/crash courses of two weeks or more are also available.
USMLE qbanks and step guides
The USMLE is taken in three separate Steps, each with its own format, timing, and passing standard. Choose your Step below:
- USMLE Step 1 Exam Guide — one day, about 280 MCQs in fourteen 30-minute blocks (about 8 hours); reported as pass/fail. Practise with our Step 1 Qbank of 3000+ high-yield questions.
- USMLE Step 2 CK Exam Guide — one day, about 316 MCQs in sixteen 30-minute blocks (about 9 hours); passing score 218. Practise with our Step 2 CK Qbank of 2000+ high-yield questions.
- USMLE Step 3 — two days: about 232 MCQs on Day 1 (about 7 hours), then about 180 MCQs plus 13-14 computer-based case simulations (CCS) on Day 2 (about 9 hours); passing score 200. The USMLE recommends completing at least one year of US postgraduate training before taking it.
Step 1 has been reported as pass/fail since January 2022, so Step 2 CK is now the main numeric score residency programs see. The Step 2 CK passing standard rose from 214 to 218 for exams taken on or after July 1, 2025.
How the USMLE works
All three Steps are computer-based, closed-book exams taken at Prometric test centers, and you have a maximum of four attempts at each Step (see our USMLE retake rules). If you are eligible, Step 1 and Step 2 CK can be taken in any order, but Step 3 is only available after you pass both. See our USMLE timeline for when to take each Step.
International medical students and graduates are eligible for Step 1 and Step 2 CK if they are enrolled in, or have graduated from, a medical school listed in the World Directory of Medical Schools as meeting ECFMG eligibility requirements. Since January 2026, IMGs register for these exams directly through the FSMB via the USMLE website, rather than through ECFMG. Scores are usually released within four weeks of your test date, though you should allow up to eight. Doctors from Pakistan can read our guide to USMLE eligibility for Pakistani doctors.
To enter an accredited US residency program, IMGs also need ECFMG Certification: passing Step 1 and Step 2 CK, plus meeting the clinical and communication skills requirement through one of the ECFMG Pathways and the OET Medicine English test. Step 3 then requires a valid ECFMG Certificate. See our guides to OET Medicine for ECFMG and USMLE exam fees, and check the official USMLE and ECFMG websites before you apply, as fees and Pathway deadlines change every year.

Frequently asked questions
> What is the USMLE?
The United States Medical Licensing Examination is a three-step exam (Step 1, Step 2 CK, and Step 3) that US state medical boards use to make licensing decisions. International medical graduates also need Step 1 and Step 2 CK for ECFMG Certification before starting US residency.
> What is the passing score for each USMLE Step?
Step 1 is reported as pass/fail only. Step 2 CK requires a score of 218 (for exams taken on or after July 1, 2025), and Step 3 requires 200 (for exams taken on or after January 1, 2024).
> How many times can I attempt each USMLE Step?
A maximum of four times per Step. After four unsuccessful attempts at any Step, you are no longer eligible to apply for USMLE exams.
> Do I need ECFMG Certification, and what does it require?
Yes, if you are an international medical graduate entering a US residency program. ECFMG Certification requires passing Step 1 and Step 2 CK and meeting the clinical and communication skills requirement through an ECFMG Pathway together with the OET Medicine English test.
> Can passing the USMLE help me work in the Gulf?
Yes. Passing the USMLE Steps can exempt you from Gulf licensing exams, including the DHA exam in Dubai, the DOH exam in Abu Dhabi, and the MOHAP exam in the UAE, although clinical experience and other licensing requirements still apply. See our guides for DHA, DOH, and MOHAP for details.
> How should I prepare for the USMLE?
Most candidates combine a structured review of their Step's content with timed practice on a Qbank of exam-style MCQs and explanations. INTERFACE's Step 1 and Step 2 CK Qbanks and our Live Online course (50-100 hours over 5-10 weeks) are built around the current exam format.
