Obs & Gyn Past Papers Quick Summary
Neither MOHAP nor the Saudi Commission for Health Specialties (SCFHS, the "Saudi Council") publishes past papers for its Obs & Gyn exam, and Prometric does not release exam questions. What circulates as "past papers" are candidate recalls (questions remembered after the exam) or QBanks (questions written to the exam's topics with explanations): recalls show you the style, a QBank gives you volume, explanations and timed practice.
Obstetrics and gynaecology specialists preparing for the UAE (MOHAP) or Saudi (SCFHS) licensing exam usually start by searching for past papers. It is a sensible instinct, because these exams test common clinical problems in a predictable way. But it helps to know what you are actually buying or downloading. Are there real MOHAP or SCFHS Obs & Gyn past papers?
No. MOHAP states that exam questions are not disclosed, and Prometric, which delivers both exams, does not sell preparation material or release used questions. SCFHS publishes a blueprint (the topics and their weight) but not the questions themselves.
So if someone offers you the "official MOHAP Obs & Gyn past paper", treat it with suspicion. At best it is a collection of recalls with a misleading name. At worst it is old material with wrong answers. Neither will get you through if you rely on it alone.

What is the SCFHS (Saudi Council) Obs & Gyn exam format?
SCFHS publishes a blueprint for its Saudi Licensure Examination in Obstetrics and Gynecology. The 2024 blueprint gives 200 MCQs in 4 hours, with a passing score of 65% for the Registrar level and 70% for the Senior Registrar level. An older version of this page described two papers of 100 to 125 MCQs; follow the current blueprint instead. The topic weights are:

What is the MOHAP Obs & Gyn exam format?
MOHAP's specialist exam is a computer-based MCQ test at Prometric centres, with a 60% pass mark reported as Pass or Fail and a maximum of 3 attempts per category. MOHAP does not publish the number of questions or a specialty blueprint, so treat any exact count you read online as a candidate report. The general format is on MOHAP exam pattern.
Expect single-best-answer clinical scenarios across:
- Antenatal care, hypertensive disorders, diabetes in pregnancy and fetal monitoring
- Labour, operative delivery and obstetric emergencies (haemorrhage, shoulder dystocia, cord prolapse)
- Early pregnancy problems: miscarriage and ectopic pregnancy
- Menstrual disorders, infertility, contraception and menopause
- Gynaecological oncology and screening
- Urogynaecology and perioperative care

Question 1: Infertility in PCOS
A 27-year-old woman with polycystic ovary syndrome has been trying to conceive for 18 months. Her cycles occur every 2 to 4 months and her BMI is 31 kg/m2. Her partner's semen analysis is normal, hysterosalpingography shows patent tubes, and TSH and prolactin are normal. She has started a weight-loss programme. What is the most appropriate first-line medication to induce ovulation?
A) Clomiphene citrate
B) Letrozole
C) Metformin alone
D) Gonadotrophin injections
Correct answer: B) Letrozole
Current international PCOS guidance (2023) recommends letrozole as first-line pharmacological ovulation induction for anovulatory infertility in PCOS, because it gives higher ovulation and live birth rates than clomiphene, with fewer multiple pregnancies. Clomiphene, alone or with metformin, is an alternative where letrozole is not available. Metformin alone is less effective, and gonadotrophins or laparoscopic ovarian drilling are second-line options after oral agents fail. Lifestyle change and weight loss continue alongside treatment.
Question 2: Postmenopausal bleeding
A 61-year-old woman, menopausal since 51 and not on hormone therapy, reports two episodes of vaginal spotting over the past month. Speculum examination is normal and her cervical screening is up to date. Transvaginal ultrasound shows an endometrial thickness of 9 mm. What is the most appropriate next step?
A) Reassure and repeat the ultrasound in 6 months
B) Endometrial sampling (outpatient biopsy, with hysteroscopy if the sample is inadequate or bleeding persists)
C) Start combined hormone replacement therapy
D) Total abdominal hysterectomy and bilateral salpingo-oophorectomy

Postmenopausal bleeding is endometrial cancer until proven otherwise. An endometrial thickness above 4 mm in a woman not on hormone therapy needs tissue diagnosis, usually by outpatient pipelle biopsy, with hysteroscopy and directed biopsy if the result is inadequate or symptoms continue. Waiting misses cancers, HRT would mask the problem, and surgery is planned only after histology.
