DoH Exam Required Documents for Healthcare Professionals: 2026 Guide

Healthcare professionals applying for a Department of Health (DoH) Abu Dhabi licence, formerly HAAD, have to provide certain educational and professional documents for verification and eligibility assessment. This page is the document checklist: what each profession needs, where each file is uploaded, and the mistakes that send files back. DOH Common Required Documents (All Healthcare Professionals)

The DOH Licensing Procedure Manual splits the documents between two places: the primary source verification (PSV) website and TAMM. Doctors, dentists, nurses, pharmacists and allied health professionals all start from the same core list.

Uploaded for primary source verification (DataFlow)

1. Valid passport copy.
2. Academic certificate(s): your basic degree and, for specialists and consultants, your postgraduate qualification. Scan the back of the degree as well whenever it carries stamps, seals or registration details.
3. Transcript, or residency letter for postgraduate training.
4. Experience certificate(s) for every period you claim.
5. Health licence or registration from each country where you practised.
6. Good Standing Certificate, not older than 6 months. For specialist and consultant titles it should come from the regulator in your country of employment.

Uploaded to TAMM with your licence application

1. Personal photograph.
2. Official logbook for the past 2 years, signed and stamped, for surgical specialties (procedure names and numbers).
3. Basic Life Support (BLS) card for nurses, midwives and TCAM practitioners.
4. Nomination or offer letter (appointment letter), if you have one when you apply. You do not need a job offer to start.
5. UAE nationals only: Family Book (children of Emirati mothers add the mother's Family Book and their birth certificate).

Some applicants are also asked to complete a Staff Data Form and a security clearance (e-Tarasol) barcode sheet. If TAMM or your employer asks for them, our guides explain how to fill them in. Security clearance itself is a step that comes after you pass the exam and link to an Abu Dhabi facility.

What are DOH documents Submission Tips?

• Upload clear colour scans, with all four corners visible and every stamp and signature readable.
• Make sure your name and dates match across all documents.
• Upload everything at the first submission. Documents added later cost extra and delay the report.
• If TAMM or your employer asks for the Staff Data and e-Tarasol forms, use the current versions.
• For surgeons, make sure the logbook is properly signed and stamped by the concerned authority.

Can name mismatch in documents cause DOH rejection?

Yes. A name mismatch across documents is one of the most common problems, and it causes delays and sometimes rejection. Typical examples are:

• passport spelling differences
• a different name on the degree (for example a maiden name)
• licences issued with initials or a shortened name

Keep your name, dates and identity details consistent across every document. Where a difference cannot be avoided, add a supporting document such as a marriage certificate or a name change affidavit.

Do all documents need attestation?

No. The DOH Licensing Procedure Manual states that attestation or authentication is not needed for any of the required documents, because primary source verification confirms them with the institutions that issued them. That saves you embassy and ministry visits. An employer may still ask for attested copies later for its own HR file, so keep your originals safe.

Is passport validity important during DOH application?

Yes. You need a valid passport copy for both PSV and TAMM, and the name on it is the reference for every other document. An expired or nearly expired passport can cause identity verification problems and delays, and you may have to update your details in TAMM. If your passport is close to expiry, renew it before you apply.

Are scanned copies acceptable for DOH application?

Yes, the whole application is online, so you upload scanned copies. The scans must be clear, with all corners visible, stamps readable, signatures visible and no cropped pages. Poor-quality scans are one of the commonest reasons a file comes back.

Can I update documents after submission?

In many cases, yes. DOH or DataFlow may ask for updated scans, corrected certificates or additional verification documents. Answer these requests quickly. Repeated corrections slow your approval, and a document added to PSV after submission is charged separately.

Do I need Arabic translations for documents?

Only for documents that are not in English or Arabic. DOH asks for a legal translation of any document in another language, uploaded together with the original. English and Arabic documents need no translation.
